Claims Reporting Procedures

Important Notice to our Workers' Compensation Policyholders

You can save your company money and maintain good relations with your employees by promptly reporting all job-related injuries to Mutual Assurance Administrators, Inc. Prompt reporting allows MAA to investigate the accident, establish proper control of medical management, avoid penalties being assessed against you, and make prompt and proper payment or declination. Prompt reporting will also help MAA control costs, which affect your policy premium.

The Workers' Compensation law requires prompt payment of benefits. Independent studies have shown that late reporting of injuries and delays in payment of benefits result in more litigation and higher claim costs. Higher claim costs will result in higher premium costs to you.

To assist you in making prompt and complete reports, use the outline below when an employee is injured on the job.

  1. Get immediate medical attention (you can contact the MAA Claims Department for a referral or recommended physicians). An employer has 72 hours to provide medical treatment after notice of an injury or the employee can choose his own physician.
  2. IMMEDIATELY call MAA Claims Department at (405) 848-1975 X2507 or (800) 825-3540 X 2507 for reporting injury by phone and complete the Employer's First Report of Injury Form (Form 2) and Fax, Email or mail to the MAA WC Claims Department.
  3. Complete ALL sections of the First Report of Injury report. Important areas are:
    • Name, address (including zip code), work and home phone numbers of injured employee
    • Social Security number and date of birth of the injured employee
    • Length of Employment, wage, occupation (job description) including department, number or location, or branch
    • Description and time of accident as related by the employee. Do you agree?
    • Is the employee losing time from work?
    • Names of witnesses
  4. Keep a copy of the First Report of Injury for your files and send a copy to MAA by fax, email or mail.
  5. If the claim involves a fatality, paralysis or serious injury, call the MAA WC Claims Department at (405) 848-1975 X2507 or (800) 825-3540 X2507 immediately.
  6. Remember, prompt reporting is very important. DO NOT DELAY reporting an injury because of incomplete information, lack of medical information or questions of the claim's validity. If in doubt, REPORT IT. Penalties imposed by the State Division of Workers' Compensation can result in fines up to $1,500.00 to you, the insured.
